Explanation of Our Cash-Based Direct Care Service Model

At Samantha Cohen, MD, Inc., we utilize a cash-based direct care service model, where patients are charged directly for services rendered, without going through insurance companies. This direct care model is designed to simplify the billing process and allows Dr. Cohen to spend more time with her patients and their families so that she can provide individualized, high-quality care.

1. Fee Structure: Our current rate is $600 per hour (broken down as $60 per 0.1 hour) for in office and telehealth visits. This fee covers Dr. Cohen’s clinical time spent with you and/or your child in consultation, completing developmental-behavioral assessment(s), interpreting the results of questionnaires, reviewing prior assessments/medical records, report writing/clinical documentation, treatment planning, counseling, communicating with other healthcare professionals if needed, etc. In-home assessments and school observations are also available upon request. The fees charged for group visits vary by the type of group and number of participants (see the information about the group you are interested in for more specific details regarding fees).

Fees are subject to change and may vary depending on the type of appointment.

2. Billing and Payment: Payments for services provided by our office are due in full at the time of the appointment. If your visit exceeds the allotted time designated for your appointment, you will be responsible for additional fees. The total cost of each visit is calculated based on the amount of time spent, rounded up to the nearest 0.1 hour (i.e. $60 per 6 minutes).

3. Superbills: Though we do not accept insurance, we can provide a superbill at the end of each visit. A superbill is an invoice that you can submit to your insurance company for potential reimbursement. Please note that reimbursement depends on your insurance plan's coverage for out-of-network providers.

4. Benefits of a Cash-Based Direct Care Model: This model allows Dr. Cohen to spend more time with each patient, providing comprehensive and personalized care. It also allows for greater flexibility in scheduling, enabling us to accommodate urgent appointments and provide longer consultation sessions when necessary.

We are committed to transparency and openness about our fees. If you have any questions about our cash based direct care model, your superbill, or your bill, please do not hesitate to contact our office.

SAS Small Group Program Fees:

  • $500: 2 hour Intake Interview and Information Session (with parents and child)

  • $150: Per 1 hour combined child and parent group meeting (weekly for 18 weeks + 4 follow-up meetings over the following 6 months = 22 meetings total)

  • $50: Per 30 minute virtual parent only group meetings (4 visits ~once a month during the initial 18 weeks, and one meeting 3 months after completing module 9)

  • $600: Per private (1:1) child and parent meetings

    Payment: Payments are due at the time of service (i.e. after each meeting) and are billed to the card you have saved on file (credit/debit/FSA, etc.) in your child’s Charm Health account. As noted above, Dr. Cohen is not contracted with any insurance companies, but she is able to provide superbills after each visit that families can then submit to their individual insurance company for potential reimbursement for services from an out-of-network provider.

TEACH ME ADHD Group Program Fees:

  • Head Quarters for the All Deeds Health Detection Agency handles registration and the course fees for TEACH ME ADHD.

  • The cost for the 8 week course + 6 months worth of access to the TEACH ME ADHD Members Only Community is a one time charge of $697. There is also an option for a payment plan: $250/mo x 3 payments.

  • When you join the wait list you will be notified when registration officially opens and you will get a secret code for $100 off if you register during priority registration.

  • The course is not covered by insurance, but there is a form that families can access to use FSA/HSA funds to help with covering the cost of the course. 

  • In addition to the 8 week course, membership to the TEACH Me ADHD Community includes access to Dr. Bauer’s Members Only Teach To Go podcast, Coffee Chats, recordings of the TEACH ME ADHD classes to review the course content and/or to use if you are not able to attend a live class.

    • The Teach To Go podcast features interesting guests, and has a lot of great information, so that families can continue to learn on the go.

    • The coffee chats are opportunities for parents to meet with Dr. Bauer and ask questions related to ADHD and parenting (mini-parent coaching sessions).

    • Overall, this is a great deal for everything that is included!

Good Faith Estimate Notice

In accordance with the No Surprises Act, we provide a Good Faith Estimate (GFE) to our patients for the cost of care. This estimate reflects the total expected cost for non-emergency services provided in our office, including any potential additional charges.

1. Understanding Your GFE: The GFE includes the anticipated cost of services, including an initial Developmental-Behavioral Evaluation, and Follow-up visits, is based on this practice’s standard fee-for-service hourly rate of $600/hour (patient care services are billed in 6-minute increments at a rate of $60 per 0.1 hours) for in-office and telehealth visits. In-home assessments and school observations are also available upon request. Special pricing applies for those visits. The cost for group therapy sessions varies based on the type of group and group size. The GFE is not a contract or guarantee of the final total cost. The actual cost may vary based on the specific services rendered and the individual health needs of the patient.

2. Self-Pay Patients: You are responsible for the total cost of services as outlined in the GFE.  

3. Insurance Coverage: This practice does not accept insurance. However, we can provide a superbill for you to submit to your insurance for partial reimbursement (the reimbursement amount is determined by your insurance company).

4. Billing: Your debit or credit card on file will be billed automatically at the time services are rendered. If you do not have a card on file or if the charge does not go through, you will have an opportunity to add/change the card on file or you will receive an invoice at the time services are rendered. If the actual cost is higher than the GFE, we will provide an explanation of the difference.

5. Dispute Resolution: If you believe you were billed incorrectly or have a dispute regarding the cost of care, please contact our office immediately. We are committed to resolving any financial concerns promptly and fairly.
